New Client Forms
In the first session with your therapist it is important spend more time getting to know you and less time on paperwork, so we ask that you print and fill out the following forms ahead of time. ​Please email completed forms before your scheduled appointment.
Please note: These forms are in Portable Document Format (pdf) and require a pdf reader. If your computer does not have a pdf reader, you may download Adobe Reader X for free here.
